I hereby order the services that I have checked above, for which I agree to pay all normal and customary preparation and delivery charges in full to Legal Video Associates, LLC and/or its affiliates.

A retainer may be required prior to the deposition.  If the job cancels and less than 24 hours notice is given, there is a $250.00 cancellation fee. All invoices are due and payable within 30 days of receipt.  There is a late charge of 1.5% per month added to invoices not paid within 90 days of original invoice date.  If it is necessary for us to institute action to recover any amounts due, a reasonable charge for attorneys’ fees and collection costs will be added. If deemed necessary, payment may be due upon delivery.  Payment is the responsibility of the attorney, not their client.
